Information and Instructions Regarding the QEEG Process
This is a Microsoft Word document. Its purpose is to insure that we get the maximum benefit from the QEEG process. Information on this form will go to the people in Bellevue who do the QEEG data collection and to the neurologist who will analyze the QEEG and make specific, customized treatment recommendations based upon the QEEG results.
- Using your computer, please answer the document's questions.
- Please e-mail the completed form to Robert Lawson at robert@eegworks.com and ask him to email the form to the individual who will be receiving and reviewing the QEEG data.
- Please print out two copies of the form and take a printed copy with you to your appointment with Mr. Lawson and bring a copy when you see me next.
- Please send me an email note once you have done the test so I will know to be on the lookout for the results.
